IASTM Therapy
Scar tissue and chronically tight fascia don’t always respond to hands alone. IASTM, instrument-assisted soft tissue mobilisation, uses specially designed tools to reach and break down restricted tissue more precisely than manual pressure can manage on its own.
What IASTM is
Smooth, contoured instruments (typically stainless steel or a similar rigid material) are used to apply controlled pressure and scraping motions across the skin over tight or scarred tissue. The tool’s edge lets a physiotherapist detect and target specific areas of restriction, adhesions, or fibrous tissue that can be difficult to isolate with fingers or hands alone, then work directly on that spot.
What it’s used for
- Scar tissue from previous surgery or injury that’s restricting movement
- Chronic tendon conditions such as tennis elbow or Achilles tendinopathy
- Fascial restrictions limiting flexibility and range of motion
- Post-injury stiffness that hasn’t resolved with manual therapy alone
- Muscle and connective tissue tightness affecting movement quality
How it works
The mechanical pressure of the instrument creates a controlled, localised inflammatory response in the treated tissue. That may sound counterintuitive, but this is what triggers the body’s natural healing process, encouraging the breakdown of poorly organised scar tissue and adhesions, and its replacement with healthier, more mobile tissue over the following days.
What it feels like
You’ll feel firm pressure and a scraping sensation as the tool moves across the treated area. It’s not typically painful, though areas with significant restriction or scar tissue can feel tender. Mild redness over the treated area afterward is normal and usually settles within a day.
Combined with movement and exercise
IASTM breaks down restriction, but restoring full, lasting range of motion needs the joint and muscle to actually move through that newly available range afterward. That’s why it’s paired with targeted stretching and exercise immediately following treatment, done in isolation it loosens tissue temporarily; done alongside movement work, the improvement holds.
Who it suits
IASTM is particularly useful for old injuries that never fully resolved, post-surgical scar tissue restricting movement, and stubborn tendon conditions like tennis elbow that have become chronic. If stretching and general manual therapy have plateaued, IASTM is often the next step.
How many sessions it typically takes
IASTM is usually delivered as a short course, often once or twice a week over several weeks, rather than a single visit, since breaking down established scar tissue and restoring lasting mobility is a gradual process. Most people notice a progressive improvement in flexibility and reduced tenderness over the course of treatment, tracked against your specific movement goals at each visit.
Tools we use
The instruments used are smooth-edged and specifically designed for soft tissue work, not sharp or invasive. Your physiotherapist selects the tool shape best suited to the area being treated, larger, broader edges for bigger muscle groups, more precise edges for smaller areas like around a tendon.
Getting started
If you have a longstanding area of stiffness, scar tissue, or a tendon problem that hasn’t improved with general treatment, IASTM is worth raising at your assessment. Alpha Physiotherapy is on PWD Road, Media Town, Rawalpindi. We’ll check whether it’s appropriate for your case and build it into a broader recovery plan.
