Mid Back Pain
Pain between the shoulder blades or across the mid back is a specific kind of discomfort, often described as a dull ache, tightness, or a burning sensation that worsens with prolonged sitting. The thoracic spine (mid back) behaves differently from the lower back and neck, so it needs its own focused approach.
Common causes
- Poor posture, particularly rounded shoulders and a forward head position from desk work or phone use
- Muscle tightness and tension between and around the shoulder blades
- Joint stiffness in the thoracic spine, which naturally has less mobility than the neck or lower back
- Postural fatigue from long periods of sitting without support or movement breaks
- Occasionally, referred tension from the neck or shoulders
Why mid back pain builds up gradually
Unlike a sudden lower back strain, mid back pain usually develops slowly, from sustained poor posture at a desk, on a phone, or driving, rather than one specific incident. The muscles between the shoulder blades work constantly to hold the upper body upright against gravity; when posture collapses forward for hours at a time, those muscles fatigue and tighten, and the joints in that region stiffen from lack of full movement.
What we assess
We check how your thoracic spine moves, how tight the surrounding muscles are, and how your posture holds up over time, since mid back pain that’s worse by the end of a work day is a strong clue that posture and prolonged positioning are the main driver.
Treatment approach
Manual therapy and, where appropriate, dry needling ease the immediate muscle tension and joint stiffness. From there, the real fix is retraining posture and building endurance in the muscles that support the upper back, so the pain doesn’t simply return once you’re back at your desk. Simple adjustments to how you sit and set up your workstation often make a bigger difference than people expect.
Practical changes that help alongside treatment
- Taking a short movement break every 30 to 40 minutes when sitting for long periods
- Adjusting screen height so you’re not looking down for hours
- Building shoulder blade and upper back strength, not just stretching the tight muscles
When to get it assessed
If mid back pain has lasted more than two weeks, is affecting your breathing or sleep, or comes with numbness or tingling, it’s worth a proper assessment rather than continuing to stretch it out on your own.
How it differs from lower back pain
The thoracic spine is naturally stiffer and more stable than the lower back, since it’s anchored by the rib cage, so mid back pain rarely involves the disc-related issues sometimes seen lower down. That’s generally good news: it means the underlying problem is almost always muscular or postural rather than structural, and tends to respond reliably well to the right combination of hands-on treatment and targeted strengthening.
