Condition We Treat

Shoulder Pain

The shoulder is the most mobile joint in the body, and that mobility comes at the cost of stability, which is why it’s also one of the most commonly injured. Shoulder pain can come from several different structures, so getting the right diagnosis before treatment starts matters more here than almost anywhere else.

Common causes

  • Rotator cuff issues - strain, tendinopathy, or tearing of the muscles stabilising the shoulder
  • Shoulder impingement - tendons or bursa getting pinched during overhead movement
  • Frozen shoulder - progressive stiffness and pain that severely restricts movement
  • Instability - a shoulder that feels like it might slip or give way
  • Muscle tension around the shoulder blade and upper back, often from posture or overuse
  • Post-surgical recovery following shoulder repair or replacement

Why an accurate diagnosis matters

Shoulder pain from impingement is treated very differently to shoulder pain from a frozen shoulder or an unstable joint, using the wrong approach can slow recovery or even make things worse. A rotator cuff strain often benefits from careful, progressive strengthening, while a frozen shoulder needs restoring range of motion first before strength is added back.

What we assess

We test the shoulder’s range of motion, strength across the rotator cuff muscles, and how the shoulder blade and upper back are contributing to (or compensating for) the problem, since shoulder pain is frequently linked to how the shoulder blade moves, not just the joint itself.

Treatment approach

Depending on what’s found, treatment may include manual therapy to ease stiffness and improve joint mechanics, dry needling for tight surrounding muscles, and a carefully progressed strengthening programme for the rotator cuff and shoulder blade muscles. For frozen shoulder specifically, gentle, graded mobility work is prioritised before loading the joint with strength work.

Frozen shoulder needs patience

Frozen shoulder in particular tends to improve in phases over months rather than weeks, freezing, frozen, then thawing. Physiotherapy can meaningfully speed up that process and reduce pain along the way, but expecting a full fix in one or two sessions isn’t realistic, and we’ll be upfront with you about the likely timeline.

When to get it assessed

  • Shoulder pain lasting more than two weeks
  • Pain that wakes you at night or when lying on that side
  • Weakness, catching, or a feeling the shoulder might give way
  • Progressive stiffness that’s limiting how high you can lift your arm

Returning to sport or overhead activity

If your shoulder pain involves sport, gym training, or overhead work, going back too soon before strength has genuinely been rebuilt is a common reason it flares straight back up. We build a graded return into your programme, so the load on the shoulder increases in step with its actual capacity, rather than jumping back to full activity based on how the shoulder feels on a good day.

Do you need surgery?

Most shoulder pain resolves well with physiotherapy alone, and surgery is only considered where a structural issue (a significant tear, for example) hasn’t responded to a proper course of conservative treatment. If your case looks like it may need a surgical opinion, we’ll say so directly and can point you toward the right next step, rather than continuing treatment indefinitely without progress.
